LNA - Per Diem

Immediate opening for a Per Diem LNA: The Licensed Nursing Assistant (LNA) assists individuals to remain independent in their homes by providing personal care and household assistance.

• Provide assistance with dental care, grooming and bathing.
• Provide assistance with bed pan, commode or toilet.
• Provide assistance with transfers.
• Assist with the continuation and evaluation of the plan of care.
• Provide accurate and timely documentation as required by the agency.
• Communicate any/all necessary information to the Nurse Manager and/or the patient’s primary nurse.
• Assume additional responsibilities as requested.

Knowledge/skills/experience Requirements:

• State Registration: Current N.H. State Registration as a Licensed Nursing Assistant.
• Access to operable, required amount insured motor vehicle and a valid N.H. driver's license with acceptable record of safe driving practices.
• Willing to drive own automobile unlimited miles and in all types of weather conditions.
• Possess a current malpractice policy.
• Ability to communicate effectively, both verbally and in writing, with or without the use of interpretive devices.
• Ability to pay close attention to detail and follow verbal and written instructions

Speech Therapist

Pemi-Baker Home Health & Hospice has an immediate opening for a speech therapist for homecare and hospice patients.

The speech therapist is responsible for working closely with other members of the agency team to provide quality care to our patients. It is the responsibility of the speech therapist to document all relevant information in the patient’s chart and also participate in management of the patients care.
