Services of PemiBaker Home Health Care
Home care is a general term used to describe home healthcare, social and supportive services, as well as equipment and supplies provided to individuals and families in their home and communities. The purpose of home care is to restore a patient to or maintain in the highest possible level of health, function, and comfort. Now, due to modern technology, many medical services previously available only in hospitals or an extended care facility, are safely and effectively administered at home.

Home Care is for all ages and needs, short duration or long, and may be intermittent or continuous. The patient is encouraged to remain independent and an active partner in the treatment and rehabilitation. Home care also supports the family by providing help, reassurance and support as they cope with the challenging and rewarding tasks of caring for a loved one at home. All clinical services are provided under physician orders.

Skilled Nursing Care includes geriatric, pediatric, psychiatric, adult health, and hospice care. Alzheimer's respite is available through a grant for such purposes and is allocated on a case by case basis. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. In the community, nurses provide services at blood pressure clinics, immunization clinics, and flu clinics. Nursing specialties include ostomy and diabetic support. Home safety assessments are provided to all patients. Disease process, treatments, equipment, medications, and diet consulting are some of the areas where staff helps educate patients and families. Therapy Services include Physical Therapy, Occupational Therapy, Speech Therapy and Social Work.

Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Under the supervision of a clinician, aides can provide more specialized care such as exercise routines, blood draws and simple dressings.

Homemaker Services provide the elderly with an alternative to institutional care, which is far more costly and less beneficial to our elder population. Trained and supervised homemakers provide light housekeeping, run errands, do laundry and prepare light meals in the home. Homemakers also provide friendship and emotional support for many people who live alone. This program promotes dignity and independence.
